As a customer of The Stables Dog Grooming, I understand and agree that the following information is stored on a dog grooming record sheet: details of the groom, any dislikes my dog(s) may have, my dog(s)' temperament, any medical conditions my dog(s) may have, and any lumps/bumps/warts/scars the groomer needs to be aware of. I also understand that my name, dog's name, address, and contact numbers are entered into the form to enable the groomer to contact me about appointments, to advise when my dog is ready, in case of emergency, etc.

I give my consent for The Stables Dog Grooming to store and use my information for the above-mentioned purposes. I also give my consent for The Stables Dog Grooming to take before and after pictures of my dog(s) and sometimes during the groom, which may be used for business purposes, mainly on social media and on the business page, and will be copyright of The Stables Dog Grooming.

I acknowledge that any non-returning customer record sheets are destroyed by a shredder three years from their last visit. I understand that The Stables Dog Grooming does not share any customer data or information with any third parties. I have read and understand the information on how my data is stored and used, and I agree to the terms and conditions mentioned in this agreement.
